Our Team

Our culturally and linguistically diverse (CALD) team brings extensive experience across disability support, therapy services, nursing care, and NDIS plan management. At My Connective Care, we deliver supports in line with the NDIS Practice Standards, ensuring services are person-centred, culturally responsive, and outcome-focused. We are committed to providing safe, high-quality, and accountable supports through clear communication, informed choice, and tailored service delivery that respects each participant’s cultural background, rights, and individual goals.

Jacob

Hi, I’m Jacob, the Head of Service Delivery at My Connective Care and a PhD-qualified Clinical Nurse. I oversee service delivery to ensure high-quality, safe, and person-centred care for our participants.

Tony

Hi, I’m Tony, and I handle the technical, people, and cultural side of My Connective Care, with a focus on people administration. I help keep our systems and team processes running smoothly.

Maddison

Hi, I’m Maddie, a Clinical Nurse at My Connective Care. I support service delivery and provide training to the team to ensure high-quality and safe care for our participants.

Brandy Lee

Hi, I’m Brandy, the Team Leader at My Connective Care. I support overseas client care and help ensure smooth coordination and quality service delivery.

Glaiza

Hi, I’m Glaiza, the Accommodation Manager and Team Leader at My Connective Care. I oversee the SIL home and support the team to ensure a safe, comfortable, and supportive living environment for our participants.

Hi, I’m Ken, and I handle the technical side of My Connective Care. I support our team by managing our systems and digital tools to ensure everything runs smoothly.
